Sometimes, you need a little extra care. We’ll work closely with you to get you the care you need to live your best life.

Managed long-term services and supports (MLTSS)

Some members are eligible for MLTSS. To qualify, you or your loved one will need to meet:

  • The state’s criteria for needing an institutional level of care
  • Some financial requirements

You can get MLTSS in your home. You don’t need to live in a nursing facility or other facility.

  • Comprehensive personal care at home 
  • Home-based supportive care 
  • Home-delivered meals after discharge
  • Personal emergency response system (PERS) — includes setup and monitoring 
  • Private duty nursing (adult) 
  • Residential modifications 
  • Vehicle modifications

  • Helpful resources residential services (CRS) 
  • Community transition services 
  • Mental health and addiction services 
  • Nursing facility services 
  • Respite (daily and hourly) 
  • Social adult day care
