We use access standards to make sure you receive timely health care. Your PCP must follow these standards for ensuring your access to care.

Your PCP must:

  • Be on call 24 hours a day, 7 days a week
  • Be there in person or arrange for another PCP to treat you
  • Provide enough staff to ensure proper care

Your PCP must also try to schedule you based on the following:

  • Office hours: Your PCP will be on hand at least 20 hours per week at each primary care site for planned visits. Nonurgent calls must be returned within 24 hours.
  • Emergencies (plus emergent calls): Your PCP will handle these at once or refer you to a hospital ER.
  • Urgent appointments: Your PCP will handle these the same day or refer you to an urgent care center. Urgent calls must be returned within 4 hours of the time of the call.
  • Routine appointments: Your PCP will be scheduled within 7 to 10 days of your request.
  • Health assessments: Wellness exams and physicals will be scheduled within 3 weeks of your request.
  • Early periodic screening, diagnosis and treatment: Visits and shots will follow standard guidelines and immunization schedules.

You can get services from any provider who is in our network and accepting new members.

You also have access to a Care Coordinator and a Care Team that you choose.

A Care Coordinator helps you manage your medical providers and services. They work with your care team to make sure you get the care you need.

Your Care Team includes you, your chosen legal representative, your care coordinator, your primary care provider, your LTSS Coordinator or PIHP Supports Coordinator (as applicable) and may include your family members/caregivers and others as you choose. Everyone on the Care Team works together to make sure your care is coordinated. This means that they make sure tests and labs are done once and the results are shared with the appropriate providers. It also means that your PCP should know all medicines you take so that they can reduce any negative effects. Your PCP will always get your permission before sharing your medical information with other providers.

How to get transportation services and find a Provider

If you need a ride to your appointments, call us at 1-855-676-5772 (TTY: 711), 24 hours a day, seven days a week and choose the transportation option.

You can also call MTM directly at 1-844-549-8347 (TTY: 711) Monday – Friday, 8 a.m. – 8 p.m. You must call at least 3 days before your appointment. If you need help, call your Care Coordinator or Member Services.
