Healthy Behaviors program

 

Weight management

 

Medicaid members:
 

  • Bluetooth fitness tracker for completing 3 months in a weight loss program

  • $20 after 6 months of meeting weight loss goals

 

Florida Healthy Kids members:
 

  • Bluetooth fitness tracker for signing up for a weight management program 
  • $20 after 3 months of weight management progress 
  • $20 after 6 months of weight management progress

Substance misuse

 

  • $20 after 3 months of taking active, meaningful steps toward recovery 

  • $20 after 6 months of taking active, meaningful steps toward recovery

Quitting nicotine

 

  • $20 after 3 months of actively taking part in coaching sessions

  • $20 after 6 months of actively taking part in coaching sessions

Screenings  

 

  • $20 for completing the Health Risk Assessment (once during your plan) 
  • $20 for breast cancer screening (ages 50 to 75, once a year)  
  • $20 for cervical cancer screening (ages 21 to 64, once a year) 
  • $20 for colon cancer screening (ages 45 and older, once a year 
  • $20 for a diabetic eye exam (ages 18 and older with a diabetes diagnosis, once a year) 
  • $20 for receiving a blood sugar test with an HbA1c result of less than 8 percent (ages 21 and older with a diabetes diagnosis, once a year)

 

Pregnancy and parenting 

 

  • $50 for completing the notice of pregnancy form in the first trimester (once every pregnancy) 
  • $100 for completing a fatherhood education program (once during your plan)

  • $20 for a well-child visit (ages 21 and younger, once a year) 
  • $20 for completing each nutrition counseling session (up to two). You may also receive a youth and obesity package every three months (ages 5 to 18 with an obesity diagnosis, once a year) 

 

 

Mental health  

 

  • $20 for each follow-up care visit with a mental health provider within 7 days after a hospital stay for mental illness (ages 6 and older, once a year) 
  • $20 for each follow-up care visit with a mental health provider within 7 days after an emergency department visit for mental illness (ages 6 and older, once a year) 

 

Substance misuse

 

  • $20 for setting goals in first care visit related to substance misuse recovery (ages 18 and older, once a year) 
  • $20 for completing follow-up care visits or goals related to substance misuse recovery (ages 18 and older, once a year)

Vaping, also known as e-cigarette use, is a major problem among children and teens in Florida. Vaping can harm your child’s body and brain. If you think your child is vaping and/or smoking, we can help. You can contact us to learn about programs that may help your child quit.
